2012 Symposium Online Registration One person per registration form. The Pantry accepts VISA, Mastercard, and Discover as well as PayPal, checks and money orders. PayPal payment is currently unavailable from this page. Please use the registration form below and then call Deb with your credit card information, or send payment directly from your PayPal account to deborahspantry@comcast.net. We apologize for the inconvenience. To use online registration: (1) Fill out the registration form and check off your choices and your form of payment. (2) Click "Submit Your Registration" and (3) send in your payment or call Deb to make payment by credit card over the phone. Payment through PayPal can be made by going to your PayPal account and sending payments to deborahspantry@comcast.net. Prefer to mail your registration and payment together? We've got a form for that! Registration is not complete until payment has been received. Please see Refund and Transfer policies at the bottom of this page. 2012 ONLINE REGISTRATION FORM Deborah Peterson's Pantry does not sell, give, or otherwise distribute your personal information to any organization or individual without your expressed permission. Inclusion in our handout of symposium attendees is optional. That list is not made available to anyone other than your fellow attendees. Refund Policy: Refunds, minus a $10 processing fee, will be made for cancellations confirmed no later than March 17, or if the program or workshop are cancelled before the program or workshop date. Your cancellation must be made by telephone or email. Please do not mail cancellation requests as we cannot ensure they will arrive by March 17. Participants will be notified by telephone and by email (according to contact information provided) as soon as possible if symposium or workshop is cancelled. Registrations may be transferred: If, after your registration and payment are received, you find you are unable to attend and want to give your place to another, you must contact us by telephone or email before March 17 with the recipient's information. Please do not mail transfer requests as we cannot ensure they will arrive by March 17. Transferred registrations will not be refunded to the original registrant. You and they are expected to make private arrangements for registration fee reimbursement. Processing fee is not charged on transferred registrations.
